Diabetes Collaborative
Beaufort Jasper Hampton Comprehensive Health Services
(BJHCHS) has been actively participating in the national Diabetes Health
Disparities Collaborative sponsored by the Centers for Disease Control
(CDC) and the South Carolina Primary Health Care Association (SCPHCA)
since 1999. The purpose of the collaborative is to change and improve the
healthcare delivery system design so patients will experience improved
health outcomes. Sheldon Health
Center served as the pilot
site, or population of focus (POF). Presently, all sites are
participating in Collaborative activities. When a patient with Diabetes
comes to any of the eight health centers for a medical visit, the visit
information is entered into the Patient Electronic Care System (PECS)
data registry.
The organizational aim is to sustain and spread the redesign of the
agency’s diabetes care delivery system to improve patient
self-management skills, decision support, information system, community
relationships, and leadership abilities.
Outreach Services
Beaufort Jasper Hampton Comprehensive Health Services
provide Outreach Services for the Diabetes Collaborative. These services
include monthly health fairs and community education classes (i.e.
Diabetes 101) through the Diabetes and Nutrition Counseling Office.
BJHCHS has also presented a Diabetes Control Program. Other outreach
services consist of patient enrollment in the patient assistance program,
which helps patients secure their diabetes testing supplies.
